Once you know that your child/children will be leaving Lyneham Primary, perhaps due to a house move or a posting, you will need to complete our Notification of Child Leaving form. Don't worry if you have not got all of the details of your move just yet. It is always better to fill out what you can and then update us by email/phone once you know more.

Please email completed forms back to admin@lynehamprimary.wilts.sch.uk or hand them in to the school office as soon as possible.

This form is not necessary for Year 6 leavers who are going to secondary school from LPS.

You will need to contact your new school directly to find out their admissions procedure and apply accordingly.
